If you have not already done so, please download and complete the following form before arriving to your first appointment:

 

New Patient Registration

You may hand deliver, mail, or fax your completed New Patient Packet to our office so that it is received at least one week prior to your appointment date. (If you choose to fax the New Patient Packet please call to verify whether we received it.)


This will allow us adequate time to get your information entered into our computer system and will expedite the time you are in our office for your first appointment. If your completed New Patient Packet has not been delivered to our office prior to the date and time of your appointment you may be rescheduled.


Your packet must be completed, including the insurance information section.


Please bring with you all medications you are currently taking.



Prepare for your Treatment

The following information is for reference only. If you have lost the Pre OP and Post OP instructions for your procedure, please call to verify that the following instructions apply.

 

Before Your Procedure

  • DO NOT eat or drink anything 7 hours prior to your procedure.
  • IF YOU EAT OR DRINK YOU RISK HAVING YOUR PROCEDURE RESCHEDULED.
  • Bring a driver. (If you are going to have sedation.)
  • Take ½ of your normal insulin.
  • Take your High Blood Pressure medicine and/or Heart medicine and any pain medicine.
  • STOP Blood thinners, Plavix, Coumadin, Warfarin, Aspirin 325mg, Garlic, Ginko and Fish Oil 7 days prior to procedure.

 

 

After Your Procedure

Anesthesia - If you have a General Anesthetic or Local Anesthetic with Sedation:

  • DO NOT drink alcoholic beverages, including beer, for 24 hours. Alcohol enhances the effects of anesthesia and sedation.
  • DO NOT drive a motor vehicle, operate machinery, or power tools for 24 hours after the procedure.
  • DO NOT make important decisions or sign important papers for 24 hours after the procedure.

 

Diet

  • Resume normal diet as tolerated.

 

Fever

  • If your temperature is greater than 101º F, notify your physician.

 

Activity

  • Rest at home with moderate activity as tolerated.
  • No lifting of heavy objects, weighing more than 10 pounds, for 2 days.
  • Resume normal activities as tolerated on the second day with weight lifting restrictions observed.

The list of insurance providers which we accept changes frequently. Listed below are the major insurance plans which we accept. The smaller insurance plans are not listed. If you do not see your insurer listed below, please contact our office directly to ensure that your insurance will be accepted.

  • Medicare (regular and all HMO plans)

  • Other (if your insurer is not listed, please contact our office directly)

 

We currently DO NOT participate with Accountable Health, Beechstreet, and Tricare Standard.
